INSTRUCTIONS FOR FILLING THE APPLICATION FORM 1. The Application Form Should be filled in giving all particulars required.
2. Correct Date of Birth & Caste should be given as any future alteration will not be permitted.
3. Admission will be given only on production of Transfer certificate from recognised institutions and Birth certificate Extract from the office of the Registrar of Birth or from the corporation of chennai, aolone will be accepted . Along with the original Birth Certificate, a xerox copy may also be produced, community certificate for pupil should also be produced in the case of BC,SC & ST.
4. A Passport size recent photograph should be affixed in the application.
5. The Application Should be signed only by the father. if the father is not alive,the mother should sign the form. Signature by the Guardian or by the mother can be accepted only if there is a written authorisation by the father or mother as the case may be.
6. Application which does not give full particulars as required in the Application Form will not be considered.
7. Two Stamped self addressed envelopes are to be handed over along with the application.
